What this notice covers

This notice covers jmwellness.com only. It does not cover your medical records or the care you receive at our clinic. Your health information is protected under federal law (HIPAA) and handled as described in our Notice of Privacy Practices, which you can request at the front desk or by calling 817-439-9081.

Please do not send health information through this website, social media or email.

Information you give us

This website has no forms and does not ask for your name, contact details or health information. If you call or email us, we use what you share to respond to you.

The BMI and body tools on this site run entirely in your browser. What you enter is not sent to us or stored.

Booking appointments

The Book buttons take you to Tebra, our scheduling provider. Information you enter there is collected by Tebra under its own privacy policy, not by this website.

Analytics

We use these tools to understand how people use the site, such as which pages are viewed and whether visitors click Book, Call or Get Directions. They do not tell us who you are.

• Google Analytics records pages viewed, approximate location, device and browser type, and how you arrived. Google Analytics is set up so that it does not collect names, email addresses or phone numbers from this site.
• Microsoft Clarity records clicks, scrolling and page layout so we can see where the site is hard to use. All text on the page and anything typed is hidden before it is sent.
• Google Tag Manager loads the two tools above.

These tools use cookies and similar technology. You can block or delete cookies in your browser settings, or install the Google Analytics opt-out add-on. The site still works without them.

We do not sell visitor information or use these tools for advertising.

Other services on this site

• Vimeo plays the video on the home page with its do-not-track setting turned on.
• Google Maps, Facebook, Instagram, TikTok and Google Reviews are reached only through links. Once you follow a link, that service's own privacy policy applies.

Children

This website is not directed to children under 13, and we do not knowingly collect information from them through it.
